Uses and Disclosures

SafeinHome collects and creates protected health information during the course of providing its services to, for, and on behalf of individuals. SafeinHome uses and discloses individuals’ protected health information when setting up individuals for Remote Supports Services, when providing Remote Supports Services for individuals, and when billing for Remote Supports Services. SafeinHome additionally uses and discloses protected health information with third parties with whom SafeinHome has established a HIPAA business associate agreement with respect to performing business operations. SafeinHome also may use or disclose protected health information in response to lawful requests. SafeinHome will obtain authorization from an individual or the individual’s duly appointed representative before using or disclosing that individual’s protected health information in any manner requiring such an authorization. SafeinHome does not use or disclose protected health information when unlawful to do so.

Individuals’ Rights

Individuals have the right to request restrictions on certain uses and disclosures of protected health information. SafeinHome is not required to agree to such requested restrictions, except as per limited disclosure restrictions. Individuals have the right to receive protected health information communications via alternate confidential methods. Individuals have the right to inspect and copy protected health information. Individuals have the right to amend protected health information. Individuals have the right to receive an accounting of protected health information disclosures. Individuals have the right to obtain this Notice of Privacy Practices in paper form, even if the individual agreed to receive this Notice of Privacy Practices electronically.

SafeinHome Duties

SafeinHome is required by law to maintain the privacy of protected health information, to provide individuals with notice of its legal duties and privacy practices with respect to protected health information, and to notify affected individuals following a breach of unsecured protected health information.

SafeinHome is required to abide by the terms of the Notice of Privacy Practices currently in effect.

SafeinHome reserves the right to change the terms of this Notice of Privacy Practices, making the provisions of the new Notice of Privacy Practices effective for all protected health information that SafeinHome maintains. Updates to the Notice of Privacy Practices will be available publicly on the SafeinHome website: https://www.safeinhome.com.

Complaints

Individuals believing that their privacy rights might have been violated may register a complaint with SafeinHome via the contact information below, and with the Secretary of the U.S. Department of Health and Human Services, without fear of retaliation for registering such a complaint.
